Procedures for Relinquishing Units
Custodians who decide to give up their units permanently must fill out a Transfer
Form 9-064, and notify your Agency Coordinator prior to shipping your unit. The
transfer form (9-064) and the unit, along with the accessories must be shipped
back to the Agency Coordinator. The Agency Coordinator will review the form and
the equipment. After approval by the Agency Coordinator a copy will be forwarded
to the custodian. Keep in mind that once you abdicate your unit and the program,
you will not be reinstated into the program. There will be a 3 month window for
turn ins from July 1 - September 30, 2006, no exceptions. If you miss this window,
you will not only have to wait until next year, but you will have to pay for the
rekey costs. Also all paper work (Transfer Form 9-064) must be submitted to the
Agency Coordinator within 30 days, no exceptions. Failure to comply with the
relinquishing units will be grounds for revocations of Custodialship duties and
will result in removal from total access to the Federal PPS program.
